  • No — Momentous Physical Therapy & Sports Performance is a cash-based practice.

    This allows me to give you 1-on-1, individualized care without being limited by insurance restrictions, rushed sessions, or cookie-cutter treatment plans.

    That said, I can provide a superbill upon request, which you may be able to submit to your insurance for potential reimbursement depending on your out-of-network benefits

  • All clients start with a Performance Rehab Evaluation ($175).

    Typically, clients start with 1–2 sessions per week, then gradually taper as they build independence and confidence.

  • No — I do not offer pay-per-visit sessions.

    This is intentional.
