No — Momentous Physical Therapy & Sports Performance is a cash-based practice.
This allows me to give you 1-on-1, individualized care without being limited by insurance restrictions, rushed sessions, or cookie-cutter treatment plans.
That said, I can provide a superbill upon request, which you may be able to submit to your insurance for potential reimbursement depending on your out-of-network benefits

All clients start with a Performance Rehab Evaluation ($175).
This is a 1-on-1, in-depth session where we:

Based on your goals and what we find, we’ll walk through the best plan moving forward.

Typically, clients start with 1–2 sessions per week, then gradually taper as they build independence and confidence.
The goal isn’t to keep you in PT forever —

No — I do not offer pay-per-visit sessions.
